    • Major Care (crowns, root canals, dentures): Often a 12-month waiting period.

  • How They Work: PPO plans have a network of “preferred” dentists.7 You will receive the best benefits and lowest out-of-pocket costs when you visit an in-network dentist. However, the key feature of a PPO is the flexibility to visit an out-of-network dentist and still receive some benefits from your plan, though your share of the cost will be higher.8
  • Cost Structure: They typically use the 100-80-50 coinsurance structure with an annual deductible and an annual maximum benefit.
  • Best For: Individuals and families who want the freedom to choose any dentist or who have a trusted dentist they want to keep, even if they are not in a specific network.

  • How They Work: You must use a dentist who is in the plan’s specific network.9 You choose a Primary Care Dentist (PCD) from a list, and that dentist manages all of your care.10 Out-of-network care is generally not covered at all, except for true emergencies.
  • Cost Structure: These plans often have very low or even $0 monthly premiums. There is typically no annual deductible and no annual maximum benefit.11 Instead, you pay a fixed, predictable copayment for each service you receive, according to a schedule provided by the plan.12
  • Best For: Individuals and families on a tight budget who are looking for predictable costs and are comfortable choosing a dentist from within the plan’s network.

C. Dental Discount or Savings Plans: The Non-Insurance Alternative
